Previously we were using recess to build bootstrap base as it was what
was used by the original project. But recess is no longer maintained.

Now we use grunt for building js, it makes sense to use it for less too.

(If you really don't want to use grunt, you almost certainly can just
use lessc -x as it uses the same less.js on backend).

This responsive image behaviour prevents setting of image sizes in text editors,
as well as a string of pix icon bugs / profile image bugs across Moodle.

We are switching to Bootstrap 3 syntax where you need img-responsive class to images
that want this behaviour.

Includes:
* each plugin has own thirdpartylibs.xml file
* added missing libraries
* fixed existing library infos
* new Site administration / Development / Third party libraries page

The filename generated.css comes from moodle.less - so this is more obvious.

Fix minor whitespace issue in thirdparty lib (will remove exclusion for it from ci).
Update yui-gallery modules location in readme.
Remove code-coverage files.

Also minor formatting cleanup.
